What is the RN Wellness Check-In?

The RN Wellness Check-In is a virtual monthly wellness touchpoint designed to give families additional visibility and reassurance. During each session, a licensed registered nurse connects with the client and/or family to discuss general wellness concerns, review changes in daily routines, and identify non-emergency issues that may need further attention.

This service is intended to complement Ommpo Care’s oversight model by adding a professional, compassionate layer of observation and support. It helps families stay informed, feel more confident, and take action earlier when something feels off.

Ommpo Care was built on a simple belief: aging at home should feel safe, supported, and stable, not uncertain or overwhelming.

After years of working behind the scenes in home care, property management, and family support systems, we saw the same problem repeatedly: families were doing their best, but without the visibility, coordination, or protection needed to truly keep a home stable. Care was happening, services were being scheduled, and bills were being paid, yet no one was overseeing the full picture. That gap often left families vulnerable to risks they didn’t see until it was too late. So we built Ommpo Care to change that. We created an independent oversight platform designed to bring clarity, accountability, and peace of mind into the home, helping families stay ahead of the issues that too often disrupt safety, finances, and independence.

What’s included in the RN Wellness Check-In?
Each RN Wellness Check-In is structured to be simple, supportive, and easy to understand.

Included Items List:
• One scheduled virtual wellness check-in each month
• Conversation with the client and/or family member
• General review of changes in daily wellness, routine, and concerns
• Discussion of non-emergency observations that may require follow-up
• Supportive wellness guidance and family education
• A summary of general observations and suggested next steps
• Coordination notes that can support the broader Ommpo Care oversight experience
What will the RN Discuss?
Each session is conversational and supportive, but may include discussion around:

• Changes in energy or fatigue
• Appetite or hydration concerns
• Mood, confusion, or unusual behavior
• General mobility or safety concerns
• Changes in routine or level of independence
• Family concerns about overall wellness
• Questions that may need to be raised with a physician or licensed medical provider

What is The Ommpo Care RN Wellness Check-In?
The Ommpo Care RN Wellness Check-In is intended as a supportive, non-emergency wellness oversight service. It is not a substitute for medical care, diagnosis, treatment, case management, home health services, emergency response, or direct clinical monitoring.

If a client is experiencing a medical emergency, families should call 911 immediately. Any medical questions, diagnoses, treatment decisions, or urgent concerns must be directed to the client’s physician or appropriate licensed healthcare provider.
How does the RN Wellness Check-In Work?
Step 1: Add the service

Families can add the RN Wellness Check-In to an eligible Ommpo Care plan.

Step 2: Monthly virtual session

A licensed RN conducts a scheduled virtual check-in with the client and/or family.

Step 3: Wellness observations shared

The family receives general feedback, observations, and suggested next steps for awareness and follow-up.

Step 4: Continued peace of mind

With monthly touchpoints in place, families gain stronger visibility and a greater sense of support.
Is this medical care?
No. The RN Wellness Check-In is a non-emergency wellness oversight service intended to provide general support, observations, and guidance. It does not replace medical care or home health services.
Is the visit in person?
No. This service is fully virtual.
Will the RN diagnose or treat anything?
No. The RN does not diagnose, prescribe, or provide treatment through this service. Any medical concerns should be directed to the appropriate healthcare provider.
Can family members join the call?
Yes. Family participation is encouraged when appropriate.
What happens if the nurse notices a concern?
The RN may share general observations and encourage the family to follow up with the client’s doctor, emergency services, or another appropriate provider depending on the nature of the concern.
